The Post looked at audits, government reports, criminal cases and civil lawsuits in 21 states from 2000-2012. Not every incident examined is included here.
- In the case of inmate lawsuits, only federal cases are included, and only when an attorney agreed to represent the inmate or the suit survived initial motions to dismiss. Inmate suits not meeting that criteria were used to augment reports. Most of the lawsuits included here were settled for undisclosed terms.
- In the case of riots, The Post included disturbances resulting in death or serious injury, damage of at least $100,000, or those involving 100 or more inmates.
- In the case of inmate deaths, suicides or injury, The Post considered only those where the incident was linked to prison policy, mismanagement or staffing.
