Universal Property and Casualty Insurance Co. has asked for a hearing with the state’s Division of Administrative Hearings to contest a state order last month that includes a $1.26 million fine, a state spokeswoman said. The order cites alleged company failures to comply with state regulations on paying claims, denying policies and other matters.
